Parish Parents Association (PA) Reimbursement and Deposit Procedures
NOTE: Sales tax will not be reimbursed!! Parish is a tax-exempt organization and is not required to pay sales tax. Use a Tax Exempt form when you make purchases to avoid paying sales tax. Contact your committee chair to obtain a copy of the Tax Exempt form. You are responsible for any sales tax paid.
1. Request for Reimbursement Forms (PDF) To request reimbursement for an approved expense that was paid by committee chair or members. This form must be completed, receipts attached, and submitted to the VP-Finance within 30 days of the date of the purchase unless the VP-Finance has given prior approval in writing. Committee chair must sign the form to show approval of the expense before submitting the request. Committee chair must indicate the budget account to which the expense will be charged. Put the request with receipts attached in the VP-Finance file in the Parents Association credenza at either Hillcrest or Midway. NO RECEIPT, NO REIMBURSEMENT, NO EXCEPTIONS
2. Check Request Form (PDF) For checks to be written directly to vendors If available, attach an invoice or quote from the vendor that shows item description and price. Put your Check Request form with committee chair approval in the VP-Finance file in the PA credenza at either campus a minimum of 10 days before the check is needed. Checks will be returned to the credenza or mailed 10 days after submission. Emergencies: Contact VP-Finance directly to work out details. Please be considerate, and keep emergencies for true emergencies.
3. Check/Cash Register Form This two-page form is available in the PA credenza at the Hillcrest and Midway campuses. Complete this deposit form when you have received cash or checks that need to be deposited. Deliver the cash and/or checks to the VP-Finance with the white original attached. Keep yellow copy for your records.
4. Notice of Donation Form (PDF) Complete and submit to Amy Ariagno when goods, services or funds with a value greater than $100 are donated to a Parish PA activity. Note that a receipt for income tax purposes will be provided to the donor at their request.
